Utah requires a state license from the Division of Professional Licensing (DOPL) to practice massage therapy, issued through a tiered system: Massage Therapist is the full credential, with lower-requirement Massage Apprentice, Massage Assistant, and Massage Assistant In-Training classifications beneath it. Massage Therapist applicants must pass the MBLEx; a separate law-and-rule exam applies to the Apprentice tier. Effective October 1, 2025, Utah also began requiring separate state registration for massage 'establishments' (the physical locations where massage is performed), a distinct new requirement layered on top of individual practitioner licensure.

Licensure by Endorsement
Massage Therapists currently licensed in another U.S. state, territory, or district may qualify for licensure by endorsement. Requirements include fingerprint cards/background check, a request to FSMTB or NCBTMB to transfer national exam results to the Division, and verification of massage therapy education or apprenticeship completion.
Local & City Requirements
Under Utah's new massage-establishment registration law (H.B. 278, 2025 General Session, effective Oct. 1, 2025), an owner registering a massage establishment with DOPL must separately 'provide a city or county business license, if required at that location' — meaning DOPL's own registration process explicitly contemplates and requires compliance with local business licensing on top of the state establishment registration.
