MBLEx Exam Prep: Format, Cost & How to Register

What Is the MBLEx?

The MBLEx provides a consistent, nationally recognized entry-level standard that state regulators use to evaluate whether a candidate can practice massage safely and competently. It’s owned and administered by the Federation of State Massage Therapy Boards (FSMTB), a nonprofit made up of state massage therapy regulatory boards and agencies across the US and its territories — not by any individual state government directly. The exam itself is delivered exclusively through Pearson VUE professional testing centers.

Most, but not all, states rely on the MBLEx as their licensing exam. A handful run their own state-specific exam instead, and some require an additional state jurisprudence exam alongside a national one. See which states accept the MBLEx below before you register.

Exam Format & Scoring

The MBLEx is 100 questions, and you’re given 110 minutes to complete them. Your full appointment at the testing center runs about 2 hours total once you add time for a security/confidentiality agreement at the start and a short survey at the end. Content covers areas such as anatomy & physiology, kinesiology, pathology, client assessment, therapeutic massage application, ethics, and business practices — FSMTB publishes the full content-area breakdown in its Exam Content Outline.

Results are reported pass/fail. FSMTB does not publish a specific numeric passing score or scoring scale on its public candidate-facing pages, so this page doesn’t invent one — if you need the exact scoring methodology, consult the current MBLEx Candidate Handbook linked in the sources below. There’s no official limit on the number of times you can retake the exam (FSMTB notes your state may impose its own limit), and you can submit a new application immediately after a failed or missed attempt — FSMTB typically processes new applications within five business days.

Cost & How to Register

The current MBLEx fee is $265, and the retake fee is the same $265 — there’s no discount for a second attempt. Fees are non-refundable and non-transferable once your application is submitted; veterans may be able to explore VA reimbursement options separately.

1. 1. Create an FSMTB exam account

   Create a candidate account through FSMTB's online portal and log in with your username and password.

2. 2. Complete and submit the MBLEx application with payment

   Fill out the MBLEx application and submit the exam fee — applications aren't considered complete until payment is received. Fees are non-refundable and non-transferable.

3. 3. Have your school verify your education

   Your massage therapy program verifies your credentials directly with FSMTB, confirming enrollment in (or graduation from) a program covering all MBLEx content areas.

4. 4. Receive your Authorization to Test (ATT)

   Once your application and education verification are approved, FSMTB emails your Authorization to Test.

5. 5. Schedule your exam with Pearson VUE

   Book your appointment through the Pearson VUE online scheduler or by phone at an authorized Pearson VUE Professional Center. Plan to arrive 30 minutes early — late arrivals forfeit the appointment.

Finding an MBLEx Practice Test

FSMTB doesn’t publish an official free practice test, so most candidates prepare with a third-party MBLEx practice test and study guide instead. A good practice test should mirror the real exam’s 100-question, multiple-choice format and cover the same content areas — anatomy & physiology, kinesiology, pathology, client assessment, and ethics/business practices among them — so you get an honest read on where you’re weak before spending $265 on the real thing. MBLEx vs. NCBTMB: Not the Same Credential

It’s easy to conflate the two, but the MBLEx and NCBTMB certification are different things from different organizations. The MBLEx is FSMTB’s state licensing exam — the one most state boards require (directly or as an accepted option) before you can legally practice. NCBTMB(the National Certification Board for Therapeutic Massage & Bodywork) is a separate nonprofit that offers its own voluntary national board certification, with its own exam and its own continuing-education requirements, distinct from any single state’s licensing process. A handful of states accept NCBTMB board certification as an alternative pathway toward licensure, but that acceptance varies by state and is never a universal substitute for passing your state’s required licensing exam — check your specific state’s page rather than assuming.
